The Ubuntu website only has instructions for machines with Gnome or KDE installed. I've got a pure server install. I tried: sudo apt-cdrom add. If are on a computer that doesn't have network access or if you have are on a slow connection, it's easier and faster to upgrade using a disc or an ISO image file. In order to perform the udpate you need the alternate CD that you can instructions on how to perform the upgrade on the official ubuntu site.

Any time a setting is changed which alters a repository setting the 'Reload' button should be selected to allow the applicable repository database to be updated. Repository information will not normally be updated until the 'Reload' button is selected. If you do not wish to use 'Reload', select 'Close' to exit without updating the database. The 'Revert' button erases changes made since the last save. It merely cancels pending changes which have not been executed, returning the selections to their rom cd Updating using ubuntu state.

What are Repositories?
It works up to the point where do-release-upgrade just says 'No new releases found'. Perhaps I missed something? I did add the CD via apt-cdrom add and commented out all other entries. I've had apt-get dist-upgrades go bad in the past hence my original suggestion and sure enough, performing a dist-upgrade has broken things.
Probably some other things that have been left behind too. Using the recommended server upgrade instructions network works so not sure what to tell ya. Perhaps alternate would work rom cd Updating using ubuntu the do-release-upgrade option but i don't have time to test it now. I guess they assume your server will be online.
